Long-lasting high body temperature and delayed muscle breakdown in ecstasy poisoning: a case report

Abstract

A peak creatine kinase (CK) level of 409,440 U/L was recorded in a chronic ecstasy user who survived severe intoxication.

  • The patient exhibited immediate hyperpyrexia (106.5°F) and altered mental status following ecstasy ingestion.
  • Delayed onset of rhabdomyolysis and disseminated intravascular coagulation (DIC) occurred alongside prolonged hyperthermia lasting 120 hours.
  • Despite the high CK level, which is the second largest recorded in a survivor, the patient returned to baseline health after 8 days.
  • Maintenance sedation with propofol effectively resolved hyperpyrexia, while carvedilol was used to stabilize temperature after prolonged hyperthermia.
