Individuals with treatment-resistant depression (TRD) experience substantial morbidity and elevated suicide risk despite at least two adequate antidepressant trials.

  • Psilocybin-assisted therapy may offer a distinct intervention for TRD by activating certain serotonin receptors and providing psychological support.
  • Early randomized trials indicate rapid reductions in depressive symptoms and encouraging short-term remission rates, primarily in major depressive disorder patients.
  • Evidence for the effectiveness of psilocybin in treatment-resistant populations is emerging but remains limited.
  • Current research is constrained by small sample sizes, short follow-up periods, and challenges in maintaining blinding.
  • The sustainability of benefits from psilocybin therapy and the implications of repeated dosing are still not well understood.
