Ethical questions about using psychedelics in psychiatric treatments

Updated

Abstract

Essence

Psychedelic-assisted psychiatric treatments raise recurring ethical concerns across safety, consent, relationships, access, research, context, and societal impact.

Evidence

This systematic scoping review searched MEDLINE, PsycInfo, CINAHL, HeinOnline, and PsycArticles through June 2025 for peer-reviewed human psychiatric-treatment literature.

Caveat

It maps and codes ethical issues descriptively, so it does not determine their prevalence, severity, or best policy response.

What this is

  • This scoping review examines ethical issues surrounding psychedelic-assisted treatments in psychiatry.
  • It covers a range of ethical concerns identified through systematic searches of peer-reviewed literature.
  • The review maps out seven key themes, including safety, , and to treatments.

Essence

  • Psychedelic-assisted treatments raise significant ethical questions that must be addressed before widespread implementation. This review identifies seven key themes related to these ethical issues, providing a framework for future discussions and research.

Key takeaways

  • Safety and patient well-being are paramount concerns in psychedelic therapy. Vulnerable patients may experience emotional distress and exploitation, necessitating strict safety regulations and supportive therapeutic environments.
  • is complicated by the unpredictable effects of psychedelics. Clear disclosure of risks and assessment of decision-making capacity are essential to ensure participants understand and agree to the therapy.
  • Equity and access are critical to prevent vulnerable populations from being excluded from psychedelic-assisted therapies. Barriers such as financial constraints and literacy issues must be addressed to ensure fair access.

Caveats

  • The review is primarily cursory, lacking in-depth analysis of specific ethical issues. This limits its ability to prioritize the most pressing ethical concerns in psychedelic bioethics.
  • The focus on peer-reviewed literature may exclude relevant discussions found in gray literature, potentially overlooking important ethical considerations.

Definitions

  • informed consent: A process ensuring that patients understand the risks and benefits of a treatment before agreeing to participate.
  • equity in access: The principle that all individuals, regardless of background, should have fair access to healthcare resources and treatments.
