Lung-related side effects linked to GLP-1 receptor agonist medications: a review of respiratory safety concerns

Abstract

Upper respiratory tract infections were reported in 66.7% of studies assessing respiratory adverse events associated with GLP-1 receptor agonist therapy.

  • The most frequently reported adverse event was upper respiratory tract infections, occurring at rates comparable to controls.
  • Pharmacovigilance analyses indicated disproportionate reporting signals for dyspnoea and asthma-like events, particularly with exenatide.
  • Serious adverse events linked to GLP-1 receptor agonist exposure included anaphylaxis with bronchospasm and acute respiratory distress syndrome.
  • A large retrospective cohort study suggested a potentially meaningful reduction in pneumonia incidents among GLP-1 RA users compared to DPP-4 inhibitors.
  • Evidence for serious respiratory events was rated very low certainty due to the limited data and high risk of bias in pharmacovigilance studies.
