Using detailed CT scans to measure airway disease in COPD and how these relate to lung function, symptoms, and doctor assessments

Abstract

Quantitative CT measurements significantly correlated with physiological indices in 188 patients with chronic obstructive pulmonary disease (COPD).

  • The expiratory to inspiratory ratio of mean lung density (E/I-ratio) showed the strongest correlations with airflow measures, including FEF25-75% and specific airway conductance (sGaw).
  • In patients with mild emphysema, subsegmental and segmental airway wall area percentage (WA%) correlated strongly with FEF25-75% and sGaw.
  • Relative inspiratory to expiratory volume change (RVC) emerged as an independent predictor of the BODE index, with a correlation coefficient of 0.468.
  • Gas trapping measurements, like E/I-ratio, were found to correlate better with physiological indices for airway disease compared to airway measurements such as WA% or luminal diameter (LD).
  • In mild emphysema, airway measurements demonstrated stronger correlations with physiological indices than gas trapping measurements.
