Severe Intestinal Blockage in a High-Risk Patient Taking Tirzepatide Long-Term

A 61-year-old woman with diabetes and obesity on tirzepatide experienced severe small bowel obstruction following progressive constipation.

  • Tirzepatide is associated with improvements in hemoglobin A1c, significant weight loss, and enhanced liver function in patients with type 2 diabetes and obesity.
  • The patient developed progressive constipation, with only one to two bowel movements per week, after starting tirzepatide therapy.
  • Imaging revealed a closed-loop obstruction due to adhesive disease and an internal hernia, necessitating surgical intervention.
  • Pathologic evaluation showed ischemic changes in the resected bowel, suggesting potential complications related to gastrointestinal motility changes.
  • The case indicates that tirzepatide-induced motility changes may exacerbate constipation and lead to obstructive complications in susceptible individuals.
