SGLT2 inhibitors and incretin-based treatments linked to dementia risk in type 2 diabetes

Updated

Abstract

Essence

In people with type 2 diabetes, SGLT2 inhibitors were associated with lower dementia risk than incretin-based therapies, especially in older adults.

Evidence

This systematic review and meta-analysis identified 9 studies and quantitatively pooled cohort studies comparing SGLT2 inhibitors with incretin mimetics, including DPP-4 inhibitors and GLP-1 receptor agonists, for dementia incidence in type 2 diabetes.

Caveat

The pooled effect comes from observational cohort data rather than randomized dementia trials, so residual confounding limits causal claims.

What this is

  • This research evaluates the effects of sodium-glucose cotransporter 2 inhibitors (SGLT2is) on dementia risk in patients with type 2 diabetes (T2D).
  • It compares SGLT2is with incretin-based therapies, such as dipeptidyl peptidase-4 inhibitors (DPP-4i) and glucagon-like peptide-1 receptor agonists (GLP-1RA).
  • The analysis includes nine studies and focuses on the incidence of dementia and its subtypes.

Essence

  • SGLT2is significantly reduce the risk of dementia compared to incretin-based therapies in patients with T2D. Empagliflozin shows the strongest protective effect.

Key takeaways

  • SGLT2is lower the overall dementia risk with a hazard ratio (HR) of 0.82 compared to incretin mimetics. This indicates a significant reduction in dementia incidence.
  • SGLT2is demonstrate even stronger effects against vascular dementia (HR 0.49) and Alzheimer's disease (HR 0.68) compared to DPP-4i, suggesting specific neuroprotective benefits.
  • Subgroup analyses reveal that patients aged over 65 years benefit most from SGLT2is, indicating a potential age-related advantage in cognitive protection.

Caveats

  • The study relies on observational data, which limits the ability to establish causal relationships between SGLT2i use and reduced dementia risk.
  • Only cohort studies were included in the meta-analysis, which may affect the robustness of the findings due to variability in study designs.
  • Potential publication bias was assessed but should be interpreted cautiously due to the small number of studies included.
