Skeletal Muscle and Its Response to Heart, Kidney, and Metabolism Treatments in Vulnerable Patients: SGLT2 Inhibitors, Incretin-Based Drugs, and Challenges in Personalized Care

Updated

Abstract

SGLT2 inhibitors and incretin-based therapies are associated with modest weight loss and distinct effects on skeletal muscle.

  • SGLT2 inhibitors may induce modest weight loss while raising questions about skeletal muscle preservation.
  • Incretin-based therapies achieve profound weight loss, but concerns about reductions in lean tissue and functional consequences exist.
  • Current evidence suggests that both therapeutic classes preferentially reduce fat tissue, with lean tissue loss proportional to weight loss.
  • Reductions in lean tissue do not consistently indicate treatment-induced sarcopenia, as muscle strength and physical performance are often not meaningfully impaired.
  • Populations at higher risk for skeletal muscle dysfunction are underrepresented in available clinical trials.
