Starvation ketoacidosis linked to tirzepatide weight loss treatment in a non-diabetic East Asian woman

A 21-year-old woman lost 21 kg after initiating tirzepatide treatment and developed starvation ketoacidosis.

  • Starvation ketoacidosis can occur in non-diabetic individuals using tirzepatide, especially with reduced caloric intake.
  • The patient experienced significant gastrointestinal symptoms, including nausea and vomiting, leading to decreased food consumption.
  • Laboratory results showed hypoglycemia and severe metabolic acidosis, along with elevated ketone levels.
  • Treatment with intravenous glucose fluids resolved metabolic acidosis within 12 hours without the need for insulin.
