Nerve Pain Causing Chest and Abdominal Symptoms After Blood Sugar Drops with Tirzepatide Treatment

Updated

Abstract

A 67-year-old man with type 2 diabetes experienced a decrease in Hemoglobin A1c from 11.0% to 6.6% over seven months after starting tirzepatide.

  • Treatment-induced neuropathy of diabetes (TIND) is an uncommon complication linked to improved blood sugar control.
  • The patient developed severe thoracoabdominal neuropathic pain and abdominal wall protrusion after significant glycemic improvement.
  • Symptoms included burning, stabbing, and electric shock-like pain, along with dysesthesia, hyperesthesia, and abdominal wall weakness.
  • Electrodiagnostic testing revealed generalized sensorimotor polyneuropathy, but further testing on thoracic or abdominal muscles was not performed.
  • The clinical features suggested thoracic radiculoneuropathy related to the glycemic improvement, though TIND could not be definitively confirmed.
