Abstract
Current evidence supports a monitoring-based approach to 4-pillar therapy in chronic kidney disease (CKD) G4.
- The 4-pillar therapy includes renin-angiotensin system inhibitors, sodium-glucose cotransporter 2 inhibitors, the mineralocorticoid receptor antagonist finerenone, and glucagon-like peptide 1 receptor agonists.
- Landmark trials primarily involved participants with estimated glomerular filtration rates of approximately 20-30 mL/min/1.73 m², leading to uncertainty in CKD stages G4-G5.
- An implementation gap exists in advanced CKD, characterized by missed therapy initiation, interruptions during complications, and failure to reinitiate treatments post-acute triggers.
- The review integrates various analyses, including subgroup data from trials and observational studies, to assess the applicability of therapies in CKD G4-G5.
- In CKD G5, treatment decisions are more individualized and require careful extrapolation from existing evidence.
