Treating Late Leaks After Sleeve Surgery with Laparoscopic Stitching and Switching to Gastric Bypass

Abstract

An 18-year-old woman with a BMI of 44.68 kg/m² experienced a gastric sleeve leak on postoperative day 13.

  • The leak was identified as a type 2, late leak located just beyond the esophagogastric junction.
  • Operative management included laparoscopic suturing and conversion to a Roux-en-Y gastric bypass (RYGB).
  • Following the procedure, drainage of the fistula gradually decreased, with imaging showing no leak after 12 days.
  • The time to closure of the fistula from diagnosis was 1 month.
  • Repair of the sleeve leak by converting to RYGB may facilitate healing by improving drainage.
