Heart Health and Safety of Glucagon-Like Peptide-1 Drugs in Type 2 Diabetes: A Review and Analysis of Clinical Trials

Abstract

In a review of nine trials involving 69,730 participants, glucagon-like peptide-1 receptor agonists (GLP-1 RAs) reduced the risk of major adverse cardiovascular events (MACE) by approximately 14%.

  • The pooled hazard ratio for MACE was 0.86, with statistical significance (P < 0.001).
  • Eight out of nine trials favored GLP-1 RAs, with five showing statistical superiority.
  • The benefits were primarily due to reductions in myocardial infarction and stroke, while hospitalization for heart failure showed a neutral effect.
  • Gastrointestinal adverse events were the most frequently reported side effects, along with an excess risk of gallbladder and biliary disease.
  • A signal for diabetic retinopathy was observed specifically with subcutaneous semaglutide.
  • The certainty of evidence for the main cardiovascular outcomes was rated as moderate.
