Nearly all evaluated epigenetic clocks achieved excellent technical reproducibility across four independent datasets.

  • Some epigenetic clocks exhibited significant drops in technical reliability due to variations in slide position and DNA extraction protocols.
  • PCGrimAge and SystemsAge were consistently reliable across all tested conditions.
  • Biological reliability was generally lower, with most clocks showing only moderate stability under various sample collection conditions.
  • PCGrimAge was the only clock to demonstrate good biological reliability, indicated by an ICC greater than 0.75.
  • Technical reliability did not correlate with biological reliability, leading to inconsistencies in prognostic associations and response to interventions.
  • Higher biological reliability was associated with more stable outcomes in cognitive decline and diet intervention responses.
