Comparing Natural and Measurement Reliability of Epigenetic Clocks and Their Links to Disease Outlook and Treatment Response

Abstract

Most epigenetic clocks showed only low to moderate biological reliability, which is critical for their clinical application.

  • Technical reproducibility was excellent for most clocks across different DNA methylation assays.
  • Certain clocks exhibited sensitivity to experimental factors, affecting their reliability.
  • PC-based clocks, particularly PCGrimAge and SystemsAge, displayed robustness under various conditions.
  • Biological reliability was significantly lower, especially when accounting for immune composition.
  • Clocks with higher reliability were associated with more stable cognitive outcomes and consistent intervention responses.
  • Improving biological reliability is essential for enhancing the clinical utility of epigenetic clocks.
