How GLP-1 receptor activators may affect Alzheimer's disease processes

Abstract

Thirty preclinical studies indicate that GLP-1 receptor agonists may reduce Aβ levels and hyperphosphorylated tau associated with Alzheimer's disease.

  • Liraglutide consistently reduced both Aβ plaque accumulation and tau hyperphosphorylation in animal and cell models.
  • Dulaglutide demonstrated reductions in tau phosphorylation and Aβ accumulation in mouse models and showed improvements in cognitive outcomes.
  • Semaglutide had largely positive results in four studies, although one study reported no benefit regarding Aβ or tau pathology.
  • Two clinical studies showed mixed results, with a phase II trial of Exenatide reducing plasma Aβ42 but not yielding cognitive benefits.
  • A smaller trial of liraglutide did not show a reduction in Aβ burden or cognitive change but preserved brain glucose metabolism.
  • Overall, while preclinical evidence supports the efficacy of GLP-1R agonists, clinical evidence remains limited and inconclusive.
