Incretin-Based Treatments for Obesity-Related Heart Failure With Normal Pumping: Potential Benefits Beyond Blood Sugar Control

Abstract

Nine studies demonstrated that incretin-based therapies significantly improved heart failure-related symptoms in obesity-related heart failure with preserved ejection fraction (HFpEF).

  • Incretin-based therapies are associated with improvements in exercise capacity and quality of life for patients with obesity-related HFpEF.
  • These therapies consistently reduced inflammatory biomarkers and body weight among the studied populations.
  • Improvements were also noted in structural remodeling and congestion-related physiology.
  • Mechanistic analyses indicated potential benefits such as reductions in left ventricular mass and markers of myocardial and renal injury.
  • Current evidence suggests these therapies may influence multiple aspects of disease pathophysiology beyond just glycemic control.
  • Further long-term studies are necessary to determine their impact on remodeling reversal and cardiovascular outcomes.
