Ketoacidosis linked to tirzepatide use in a non-obese person without diabetes

Abstract

A 23-year-old woman without obesity developed euglycemic ketoacidosis after using tirzepatide.

  • Euglycemic ketoacidosis is characterized by metabolic acidosis with elevated ketone bodies despite normal or mildly elevated blood glucose levels.
  • The patient's laboratory results included a high anion gap of 25.9 mmol/L and significantly elevated levels of β-hydroxybutyrate and acetoacetate.
  • Symptoms such as nausea, vomiting, and diarrhea occurred following the second dose of tirzepatide (2.5 mg).
  • No infectious source was identified, leading to a diagnosis of euglycemic ketoacidosis.
  • Intravenous treatment with fluids and insulin quickly corrected the acidosis, and normal glucose tolerance was confirmed after follow-up.
