Lean mass loss appears substantial and proportionally similar during incretin-based and lifestyle weight-loss interventions, while resistance training may preserve more muscle.

Evidence

This systematic review and random-effects meta-analysis included 20 randomized controlled trials with 15,782 adults with overweight or obesity and body-composition outcomes measured by dual-energy X-ray absorptiometry or magnetic resonance imaging.

Caveat

Heterogeneity was moderate, and the comparison estimates summarize trial-level body-composition outcomes rather than proving which specific protein, resistance-training, or monitoring strategy best prevents lean mass loss.
