Fasting and Stomach Ultrasound Approaches Around Surgery for People Using GLP-1 Diabetes Medications: A Systematic Review

Abstract

Nineteen clinical studies suggest that specific strategies may reduce residual gastric content (RGC) in users of glucagon-like peptide-1 receptor agonists (GLP-1 RAs).

  • A 24-hour clear liquid diet was linked to lower RGC (approximately 1-2% versus 10%; odds ratio ≈ 0.13) in one retrospective study.
  • Withholding GLP-1 RAs for 7-8 days was associated with lower odds of high RGC compared to shorter withholding periods in prospective ultrasound studies.
  • Gastric point-of-care ultrasound (POCUS) effectively identified patients at increased risk of high RGC using validated thresholds (>1.5 mL/kg or presence of solids).
  • Aspiration events were rare and inconsistently reported across the various mitigation strategies evaluated.
  • The evidence indicates that while certain pre-procedural strategies may reduce RGC, the overall evidence base is limited, and a modest aspiration risk cannot be excluded.
