Factors Linked to How Well Psychedelics May Treat Depression

Abstract

A total of 48 studies were included in the review of predictors of antidepressant response to psychedelic-assisted therapies.

  • Process-level features during the dosing session are the most consistent correlates of antidepressant improvement.
  • Greater emotional breakthroughs, mystical experiences, and ego dissolution linked to reappraisal generally predict larger and more durable symptom reductions.
  • Anxiety-dominant or dysphoric states may attenuate benefits, often independent of overall subjective intensity.
  • Set and setting, particularly a strong therapeutic alliance and resonant music, predict the emergence of therapeutically significant acute experiences and subsequent clinical gains.
  • Baseline factors showed smaller and mixed effects, with PTSD comorbidity sometimes weakening outcomes and extensive prior psychedelic exposure associated with smaller gains.
  • Biological findings suggest better outcomes are linked to increased neural flexibility and plasticity, alongside peripheral changes involving neurotrophic, inflammatory, and stress response pathways.
