Kidney effects of GLP-1 receptor drugs in diabetic kidney disease: how they work and clinical findings

Abstract

Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) are associated with reductions in albuminuria and slowing of glomerular filtration rate (GFR) decline in patients with diabetic kidney disease (DKD).

  • GLP-1 RAs may improve glycemic control, body weight, blood pressure, lipid profile, and systemic inflammation.
  • In experimental studies, GLP-1 RAs demonstrated direct renal effects, including modulation of natriuresis and intrarenal blood flow.
  • Findings from randomized clinical trials indicate that GLP-1 RAs have nephroprotective potential.
  • The evidence positions GLP-1 RAs alongside sodium-glucose cotransporter-2 inhibitors (SGLT2i) and non-steroidal mineralocorticoid receptor antagonists (ns-MRAs) for managing patients with type 2 diabetes, chronic kidney disease, and high cardiorenal risk.
