Unexpected Stomach Contents in a Child Who Followed Fasting Rules While Taking Diabetes Medication: A Case Report and Surgery Considerations

A 14-year-old female patient on semaglutide had over 200 mL of residual gastric contents during a procedure despite fasting.

  • Gastric ultrasound indicated a distended antrum with fluid and particulate matter, suggesting inadequate gastric emptying.
  • Standard fasting protocols may not be sufficient for patients receiving glucagon-like peptide-1 receptor agonists.
  • Procedures were performed safely with rapid sequence induction to address the risk of aspiration.
  • Individualized risk assessment using point-of-care gastric ultrasound may improve anesthetic planning.
  • Further research is needed to establish appropriate fasting intervals and anesthetic strategies for patients on GLP-1 RA therapy.
