By month 3, the intervention group showed greater improvements in all primary outcomes, sustained through 24 months (all p < 0.001).

  • Cognitive behavioral therapy delivered via an app combined with inflammation-guided nutrition may enhance recovery after abdominoperineal resection for low rectal cancer.
  • Levels of inflammatory markers C-reactive protein and interleukin-6 were significantly reduced by day 7 (both p < 0.001).
  • Better preservation of skeletal muscle index was observed at 24 months in the intervention group (p = 0.046).
  • Two-year disease-free survival and overall survival rates were significantly improved in the intervention group (HR = 0.48, p = 0.021 for DFS; HR = 0.44, p = 0.036 for OS).
  • Results indicate potential benefits of an integrative approach within enhanced recovery pathways, warranting further investigation.
