Consensus was reached on 1,276 treatment options for bipolar disorder across 48 clinical situations.

  • A mood stabilizer is recommended in all phases of treatment.
  • Divalproex and lithium are identified as cornerstone choices for both acute and preventive treatment of mania.
  • If monotherapy fails, combining mood stabilizers is the next recommended intervention.
  • For milder depression, lithium may be used as monotherapy, with divalproex and lamotrigine as other first-line options.
  • A standard antidepressant should be combined with lithium or divalproex for more severe depression.
  • Atypical antipsychotics, especially olanzapine and risperidone, are preferred for managing mania or depression with psychosis.
