Do GLP-1 drugs change surgery risks in spine operations? A review and combined analysis

Abstract

Perioperative use of GLP-1 receptor agonists is associated with a lower risk of pseudarthrosis in adults undergoing spine surgery (OR 0.74; 95% CI: 0.55-1.00).

  • Thirteen retrospective cohort studies were analyzed, including 13,754 GLP-1 users and 17,591 controls.
  • Baseline characteristics were similar between GLP-1 users and controls, with a weighted mean age of approximately 61 years and a high prevalence of type 2 diabetes and hypertension.
  • Heterogeneity was noted in the studies evaluating pseudarthrosis, but a lower risk was observed with GLP-1 RA use.
  • No significant differences were found for other surgical and nonsurgical outcome measures.
  • There was a nonsignificant trend suggesting a higher risk of nerve injury associated with GLP-1 exposure (OR 1.57; 95% CI: 0.99-2.48).
  • Heterogeneity varied by outcome, being highest for reoperation and utilization endpoints.
