Preconception GLP-1RA or tirzepatide exposure was not linked to higher GDM risk after earlier discontinuation, but abrupt discontinuation close to conception was linked to higher risk.

Evidence

A retrospective TriNetX multinational propensity-matched cohort of women aged 18-45 without pre-existing diabetes compared preconception users with non-users and found similar GDM risk after >90-day discontinuation (16.3% vs. 16.8%; RR 0.967) but higher risk with <=90-day discontinuation (RR 1.536).

Caveat

The findings are observational and hinge on prescription timing and BMI rebound measures rather than prospective discontinuation assignment or confirmed causal mechanisms.
