Incretin-Based Treatments and Their Role in Heart Damage Caused by Doxorubicin: A Review of GLP-1 and Combined GIP/GLP-1 Therapies

Abstract

Incretin-based therapies may improve left ventricular ejection fraction by approximately 7-20 percentage points in chronic cumulative-dose doxorubicin models.

  • Thirteen rodent studies were included, with no eligible human studies identified.
  • Investigated incretin agents included liraglutide, exenatide/exendin-4, semaglutide, and tirzepatide.
  • Incretin-based therapies were associated with reductions in cardiac injury biomarkers and improvements in left ventricular systolic function.
  • These therapies may reduce oxidative stress, inflammation, and apoptosis in the context of doxorubicin-induced cardiotoxicity.
  • Findings were less consistent in models using acute single doses of doxorubicin.
  • The evidence is limited by small, heterogeneous animal studies with low-to-very-low certainty.
