THR-β Agonists Compared to Incretin Treatments for Early Metabolic Liver Disease Confirmed by Biopsy

Abstract

A total of 3,796 participants across seven randomized controlled trials were reviewed for the efficacy and safety of resmetirom, GLP-1 receptor agonists, and dual GIP/GLP-1 receptor agonists in treating metabolic dysfunction-associated steatohepatitis (MASH).

  • All active therapies showed better outcomes than placebo for at least one biopsy-anchored endpoint.
  • Resmetirom improved MASH resolution and fibrosis over 52 weeks, with significant reductions in low-density lipoprotein cholesterol and apolipoprotein B, while showing minimal weight change.
  • Semaglutide provided dose-dependent histologic and metabolic improvements, achieving dual-endpoint success in the phase 3 ESSENCE trial.
  • Liraglutide showed resolution improvements in a small trial, and tirzepatide resulted in significant weight loss alongside histological benefits.
  • Adverse events were mostly mild gastrointestinal effects, with GRADE assessments indicating low certainty for differences in histologic outcomes between drug classes.
  • High certainty favored incretin therapies for weight reduction, while moderate certainty favored resmetirom for lowering lipid levels.
