Incretin therapy was associated with lower recorded suicidality, self-harm, mortality, overdose, and substance-use outcomes in antipsychotic-treated people with psychotic disorders.

Evidence

A TriNetX retrospective matched cohort study compared 21,984 incretin-treated adults with psychotic disorders on antipsychotics against 21,984 untreated peers over 5 years, finding lower risks for suicidal ideation, suicide attempts, and self-harm.

Caveat

Because this was an observational EHR study, the lower risks may reflect residual confounding, treatment selection, or coding differences rather than a causal protective effect.
