Heart artery treatments before major vascular surgery to prevent heart attacks during surgery

Abstract

In three randomized controlled trials involving 1144 participants, preoperative coronary interventions showed very low-certainty evidence regarding their effectiveness in preventing acute myocardial infarction during major vascular surgery.

  • No significant difference was observed in the incidence of perioperative acute myocardial infarction between those receiving preoperative coronary interventions and usual care.
  • Limited evidence suggests a reduction in long-term acute myocardial infarction for participants receiving preoperative interventions plus usual care compared to usual care alone.
  • There was little to no effect on all-cause mortality in the perioperative period when comparing preoperative interventions to usual care.
  • The evidence is very uncertain regarding the impact of preoperative coronary interventions on long-term all-cause mortality.
  • Preoperative coronary interventions plus usual care may slightly reduce length of hospital stay compared to usual care alone.
  • Adverse effects related to preoperative coronary interventions were poorly reported, with some trials indicating deaths due to revascularizations.
