Ketoacidosis with Normal Blood Sugar After Unsupervised Tirzepatide Use in a Non-Obese, Non-Diabetic Woman

Abstract

A 30-year-old woman experienced euglycemic ketoacidosis after unsupervised use of tirzepatide for weight loss.

  • The woman had a body mass index of 24.8 kg/m² and no history of obesity or diabetes.
  • She self-administered tirzepatide, increasing her dose from 2.5 mg to 5 mg weekly.
  • Initial tests revealed high anion gap metabolic acidosis with a pH of 7.15 and bicarbonate level of 10.5 mEq/L.
  • Despite a normal glucose level of 4.2 mmol/L, serum ketones were elevated at 4.5 mmol/L.
  • After stopping tirzepatide and providing supportive care, the anion gap closed and ketones normalized within 36 hours.
