GLP-1 receptor agonists (GLP-1 RAs) may lead to significant reductions in total body weight but are also associated with declines in both fat and lean mass.

  • Weight loss from GLP-1 RAs includes reductions in both adipose tissue and fat-free mass.
  • Clinical relevance of lean mass decline may depend on factors like age, muscle reserve, and physical activity.
  • Low testosterone levels in men are linked to reduced lean body mass and may increase vulnerability to muscle loss during weight reduction.
  • Testosterone replacement therapy (TRT) can increase lean body mass in men with low testosterone.
  • No randomized trials have assessed the combined effects of GLP-1 RAs and TRT on body composition.
  • Routine combined therapy is not supported, but assessing androgen status may be important for men undergoing significant weight loss with GLP-1 therapies.
