GLP-1 receptor agonists may induce weight loss but are associated with common muscle mass loss in older adults.

  • Sarcopenia and sarcopenic obesity are major concerns for older adults using obesity pharmacotherapy.
  • Weight cycling could introduce uncertainty regarding skeletal muscle health and the risk of sarcopenic obesity.
  • Current data on muscle mass and physical performance effects of GLP-1 receptor agonists are limited and inconsistent.
  • Favorable effects on muscle metabolism and composition are suggested through mechanisms involving inflammation suppression and mitochondrial function.
  • Mild to moderate gastrointestinal symptoms are common adverse events but may be reduced with dietary counseling.
  • Combining GLP-1 receptor agonist therapy with tailored resistance exercise and adequate protein intake may help preserve muscle mass.
